For us to help you find the best community for you or your loved one, we will need to gather some information to get started. Here is a list of some information you can prepare before our first phone call. 

• Name of the potential resident(s)

• Age

• Do they have a Power of Attorney?

• Current health issues that may require assistance

• Can they perform their own ADLs (activities of daily living) showering, dressing, toileting etc.?

• Are they on current medications? If so, how many, and how do they currently manage them?

• Are there any cognitive issues?

• What is the main reason you are beginning this search?

• Preferred location (county, town)

• What activities, hobbies, or social outings do they enjoy?

• Timeline-how soon are you looking to make a move?

• Finances-     

  • Do they have a house to sell?

  • Do they have Long Term Care Insurance? 

  • Are they a veteran?

  • Do they have Savings/Social Security/Pension/Investments?
